Influenza H1N1 (Swine Flu): Symptoms, Spread, Antigenic Drift & Shift

Context:

Recently, Delhi has witnessed a sharp rise in Influenza A(H1N1) cases. According to National Centre for Disease Control (NCDC) data cited in the article, 1,449 cases were reported up to August 12, compared with 229 cases during the corresponding period last year.

What is Influenza H1N1?

      • Influenza viruses belong to the Orthomyxoviridae family. H1N1 is a subtype of Influenza A, named after two surface proteins: Hemagglutinin (H) and Neuraminidase (N). H1N1 contains the H1 and N1 subtypes of these proteins, which help the virus infect host cells and interact with the immune system.
      • The term “swine flu” became popular because the virus responsible for the 2009 pandemic contained genetic material originating from influenza viruses associated with humans, pigs and birds.

2009 H1N1 Pandemic:

India reported its first H1N1 case in Hyderabad in May 2009. In June 2009, the World Health Organization (WHO) declared H1N1 a global pandemic. The virus subsequently became established as one of the seasonal influenza viruses circulating among humans.

Why is H1N1 Spreading?

      • Influenza is a seasonal disease, with outbreaks influenced by climatic conditions, particularly during the monsoon period in India. Increased human interaction, population density and local weather conditions can facilitate transmission.
      • However, the exact reason for a sudden increase in cases requires genetic and epidemiological studies, as changes in the virus may also influence its spread and ability to evade existing immunity.

Antigenic Drift and Antigenic Shift:

      • Antigenic Drift: Antigenic drift refers to small and gradual mutations in influenza viruses. These changes can alter the HA and NA proteins, reducing the effectiveness of existing antibodies. Drift contributes to seasonal outbreaks and is one reason influenza vaccines need periodic updating.
      • Antigenic Shift: Antigenic shift is a sudden and major change in an Influenza A virus. It can produce a novel influenza subtype against which humans have little or no immunity. Such changes have the potential to cause influenza pandemics, as occurred in 2009.

Symptoms and Vulnerable Groups:

      • Common symptoms include fever, cough, headache, fatigue, muscle aches, chills and sweating. Severe infection can result in pneumonia or respiratory failure.
      • The elderly, children, pregnant women and people with conditions such as diabetes, asthma, heart disease or weakened immunity are at greater risk of severe disease.

Treatment and Prevention:

      • Treatment is mainly supportive. Oseltamivir, an antiviral medicine, can be effective, particularly when started early in appropriate cases. Antibiotics do not treat viral influenza.
      • Preventive measures include annual vaccination, hand hygiene, respiratory etiquette, adequate ventilation and avoiding close contact while symptomatic.

Conclusion:

The resurgence of H1N1 highlights the importance of genomic surveillance, vaccination, early diagnosis and One Health preparedness. Understanding antigenic drift and shift is crucial for preventing seasonal outbreaks and preparing for future influenza pandemics.
