PM Family Care Tracker: Digital Welfare Platform

Why in News?

Recently, Union Home Minister Amit Shah inaugurated the ‘PM Family Care Tracker’ pilot project in Gandhinagar on June 28, 2026. The pilot will be implemented in Gandhinagar Lok Sabha constituency and may later be scaled to Gujarat and other states. The initiative aims to strengthen real-time monitoring of maternal and child welfare delivery using an integrated digital platform.

What is the PM Family Care Tracker?

The PM Family Care Tracker is an AI-enabled integrated governance platform designed to ensure that pregnant women, mothers, and children receive timely and end-to-end benefits of welfare schemes without gaps or duplication.

It creates a life-cycle digital record by integrating:

    • Civil registration systems (birth and death records)
    • Health, nutrition, education, and social welfare databases
    • Aadhaar-linked beneficiary records (where applicable)
    • Scheme-specific databases across departments

Each child is assigned a unique digital identity, enabling tracking from pregnancy to 18 years of age.

Objectives of the Platform:

    • Ensure automatic inclusion in welfare schemes (proactive governance)
    • Strengthen last-mile delivery of benefits
    • Reduce exclusion and duplication errors in beneficiary databases
    • Enable predictive monitoring of health and education risks
    • Improve inter-departmental coordination and accountability
    • Build a real-time social protection dashboard

How Does the System Work?

The platform integrates nearly 16 flagship schemes, including:

    • Pradhan Mantri Matru Vandana Yojana
    • Janani Suraksha Yojana
    • Janani Shishu Suraksha Karyakram
    • PM POSHAN
    • Mission Vatsalya
    • Rashtriya Bal Swasthya Karyakram
    • Sukanya Samriddhi Yojana
    • PM CARES for Children
    • National Child Labour Project

Automated Alert Mechanism:

The system uses rule-based triggers and AI-driven alerts:

    • Missed immunisation immediate alert to ANM/health officer
    • School dropout risk flagged to teachers, education officers, and local bodies
    • Malnutrition indicators triggers ICDS intervention
    • High-risk pregnancy escalated to medical supervision
    • Delay in benefit delivery escalated to district/state level dashboards

Each case remains open until closure confirmation, ensuring accountability.

Key Features of the project:

    • Unique Child Digital ID linked to birth registration
    • Real-time monitoring dashboard for officials at all levels
    • Interoperable databases across ministries
    • Geo-tagging of beneficiaries and service delivery points
    • Performance indicators tracking maternal and child health outcomes
    • Automated escalation workflow system
    • Analytics-based risk prediction (e.g., malnutrition, dropout probability)

Significance of this project:

    • Strengthens Digital Public Infrastructure (DPI) for social welfare
    • Supports Direct Benefit Transfer (DBT) efficiency and transparency
    • Enables proactive governance instead of reactive welfare delivery
    • Helps reduce Infant Mortality Rate (IMR) and Maternal Mortality Ratio (MMR)
    • Enhances monitoring of Sustainable Development Goals (SDG 2, 3, and 4)
    • Improves India’s capacity for data-driven policy planning

Challenges:

    • Data privacy, consent, and protection concerns
    • Risk of exclusion due to incorrect or outdated data
